Tribulus: what it actually does, dosage, and whether it's worth it

What tribulus actually does in the body, the doses used in real studies, its effect on testosterone and libido, and when it is simply not worth your money.

Tribulus terrestris is one of the best-selling herbal supplements in gyms, almost entirely on the promise that it raises testosterone. That claim is roughly fifty years old and it still sits on the label. The problem is that what the label says and what has been measured in humans are two different things.

Where the testosterone story came from

The compound usually named on the bottle is protodioscin, a steroidal saponin. The word steroidal here describes the ring structure of the molecule, not that it behaves like an anabolic steroid in your body. The early work from the 1980s and 1990s was done in rats, rabbits and primates, at doses per kilogram of bodyweight several times higher than what people take, and much of it was never reproduced in humans.

What the human studies show

Testosterone

This part is fairly consistent. In a study of 21 young men aged 20-36 taking 10 to 20 mg per kilogram of bodyweight daily for four weeks, there was no difference in total or free testosterone compared to placebo. Trained men look the same: 450 mg per day for five weeks in rugby players changed neither the hormone profile nor strength test results. In men with normal testosterone, tribulus does not move it.

Strength and muscle mass

An eight-week trial in men doing resistance training, at roughly 3.2 mg per kilogram per day, found no difference in lean mass or strength versus placebo. Both groups improved equally, because the improvement came from the training. When someone says their squat jumped 10 kg since they started tribulus, that is almost certainly training, food and sleep, not a capsule.

Libido and erectile function

This is the one place where something shows up. In 180 men with mild to moderate erectile dysfunction, 750 mg per day split into three doses over 12 weeks produced a moderate improvement on the standard erectile function questionnaire compared to placebo, with no meaningful change in testosterone. Improvements have also been reported in women with low sexual desire at the same dose. The effect is real but modest, it does not work through hormones, and it is not comparable to prescription ED medication.

The doses that were actually used

  • For libido and erectile function: 750-1500 mg per day, split into 2-3 doses with food, extract standardised to 40-60% saponins.
  • In hormone studies: 10-20 mg per kilogram of bodyweight, which is 800-1600 mg per day at 80 kg, and still no testosterone effect.
  • Trial length: 8-12 weeks is plenty to know. If nothing has changed by week 12, week 24 will not change it either.
  • Skip the twenty-ingredient blends. You cannot tell what is doing what, and individual ingredient doses are often token amounts.

Protodioscin content varies heavily by plant part and origin. Fruit and root are not the same, and two batches with identical label claims are not necessarily identical products. That is one reason study results are hard to compare against each other.

Side effects and risks

For most healthy adults, at the doses above and over 2-3 months, tribulus is well tolerated. The common complaints are bloating, cramping and nausea, and they usually settle if you take it with a meal. There are isolated case reports of kidney and liver injury, mostly at very high doses or with homemade preparations of the plant, plus one reported case of gynecomastia. Sheep grazing heavily on it develop liver damage, which does not translate directly to a human taking 750 mg, but it is a reminder that herbal does not mean no ceiling.

If you take blood pressure medication, diabetes medication, blood thinners or cardiac drugs, check with your doctor first. Interaction data is thin, and that is a reason for caution rather than reassurance.

If you are a tested athlete

Tribulus is not a banned substance, but it has been reported to raise the urinary testosterone to epitestosterone ratio, which is one of the parameters doping control looks at. On top of that, herbal booster products are among the most frequently contaminated supplement categories. If you are subject to testing, just skip it. The possible upside is far too small for that risk.

When to see a doctor

A supplement is not the answer to a symptom that persists. Book an appointment if you have:

  • Low libido or erectile problems lasting more than 3 months, especially alongside fatigue, strength loss or low mood.
  • Morning erections that have disappeared entirely, which shifts suspicion toward a physical cause.
  • Unexplained loss of body hair, shrinking testicles, or fertility problems.
  • Pain under the right rib margin, dark urine, or yellowing of skin or eyes while taking any herbal product. Stop it and get seen immediately.

The basic workup is simple: testosterone drawn between 7 and 10 in the morning, ideally twice on separate days, plus LH, FSH, prolactin, SHBG, full blood count, glucose and thyroid.

What actually moves the needle

  • Sleep: 7-9 hours. A week of 5-hour nights drops daytime testosterone in young men by roughly 10-15%.
  • Strength training: 3-4 sessions per week, 10-20 hard sets per muscle group per week, across 5 to 30 reps.
  • Body fat: going from 30% to 20% body fat does more for your hormone panel than any plant extract.
  • Food: 1.6-2.2 g of protein per kilogram of bodyweight, and no sitting in a deficit deeper than 500 kcal per day for months.
  • Alcohol: a regular 4-plus drinks in the evening works against everything above.

The short version

  • Tribulus does not raise testosterone in healthy men. This is the best-studied part of the claim and the answer is no.
  • There is no evidence it increases strength or muscle mass.
  • The only realistic effect is a modest improvement in libido and erectile function at 750-1500 mg per day over 8-12 weeks.
  • It is generally well tolerated, but skip it if you are a tested athlete or have liver or kidney disease.
  • If symptoms last more than 3 months, get bloodwork instead of buying capsules.
