L-Carnitine: What It Actually Does, Dosage, and Whether It's Worth It

What L-carnitine actually does in muscle, which doses have been tested, how long it takes, who might benefit, and when your money is better spent elsewhere.

L-carnitine has a clear, well-documented role in burning fat — which is exactly why it gets sold as something it isn't. The gap between what the molecule does inside a cell and what the label promises is enormous. Here is what the research actually shows, which doses make sense, and when you're better off spending the money elsewhere.

What it does in the body

Carnitine is a compound your body makes on its own from the amino acids lysine and methionine, with help from vitamin C, B6, niacin and iron. Its job is transport: long-chain fatty acids can't enter the mitochondria by themselves, so carnitine ferries them across the membrane to be burned. Around 95% of your total carnitine sits in skeletal muscle — roughly 15 to 20 grams in an adult.

From food you get it mostly from meat. 100 g of beef provides about 60 to 150 mg, 100 g of pork around 25 mg, chicken only 5 to 10 mg, and a glass of milk about 8 mg. Daily meat eaters take in 60 to 180 mg per day; vegetarians and vegans usually 10 to 15 mg. Your body synthesises another 15 mg or so daily. For a healthy person eating normally that's enough — true deficiency is rare and usually tied to disease, not diet.

Why a capsule rarely raises muscle levels

This is the part the marketing skips. When you swallow 2 g of L-carnitine, blood levels spike, but very little reaches muscle. Absorption from a supplement is only 14 to 18% (from food it's 55 to 85%), and muscle only pulls carnitine in when insulin is strongly elevated.

The studies that genuinely raised muscle carnitine used about 2 g of L-carnitine alongside roughly 80 g of fast carbohydrate, twice a day, for 12 to 24 weeks. The payoff was about 20% more carnitine in muscle and about 10% more work output in an endurance test.

That's 160 g of extra carbohydrate every single day for half a year. If you're in a calorie deficit, that's completely counterproductive.

Fat loss: the realistic number

Meta-analyses of randomised trials show an average loss of about 1.2 kg of body mass over 8 to 24 weeks, with BMI dropping around 0.2 to 0.3 points. The effect is clearest in overweight participants who were also dieting, and it essentially disappears in lean, trained people.

For perspective: 1.2 kg over six months is roughly what you'd get from eating 40 to 50 fewer calories a day. A 300 to 500 kcal daily deficit produces 0.3 to 0.5 kg per week. L-carnitine is not a fat burner in the sense it's marketed as.

Recovery and muscle soreness

Here the data is more convincing. Doses of 1 to 2 g per day of L-carnitine L-tartrate over 3 to 4 weeks reduced markers of muscle damage and subjective soreness after heavy eccentric work (think 5 x 15 squats, or 4-second lowering phases). The effect is moderate and won't change what you can lift, but you notice it a day or two later.

For maximal strength, VO2max, and acute fat oxidation during training, there's no reliable effect in already-trained people.

Doses that have actually been tested

  1. Recovery and soreness: 1 to 2 g per day in a single dose, for at least 3 weeks before you expect a difference.
  2. Raising muscle carnitine: 2 g twice daily with about 80 g of carbohydrate per dose, for 12 to 24 weeks. Expensive, annoying, rarely worth it.
  3. Acetyl-L-carnitine (fatigue, cognitive complaints): 500 to 2000 mg per day, usually split into two doses.
  4. Adults over 65 and people with chronic fatigue: 2 g per day over 8 to 24 weeks produced a moderate reduction in fatigue in several small trials.

Timing relative to your workout doesn't matter. What matters is consistency and taking it with a carbohydrate-containing meal.

Who it might make sense for

  • Vegans and vegetarians taking in 10 to 15 mg per day — their plasma levels run lower, though muscle stores are usually fine.
  • People over 65 dealing with pronounced fatigue.
  • Anyone doing a lot of eccentric work who struggles to recover between sessions.

Who it doesn't help: someone training three times a week, eating meat, sleeping 6 hours, with their nutrition unsorted. The same money spent on adequate protein (1.6 to 2.2 g per kilogram of body weight), creatine (3 to 5 g per day) and one more hour of sleep will do far more.

Side effects and interactions

Doses up to 2 g per day are generally well tolerated. Above 3 g per day you start seeing nausea, abdominal cramping, diarrhoea and a distinctive fishy body odour — harmless, but unpleasant.

Gut bacteria convert some carnitine into TMAO, a compound linked in observational studies to higher cardiovascular risk. Causation isn't established, but it's a reason not to take 4 g a day for years without need. Carnitine can also interfere with thyroid hormone action, and there are reported cases of prolonged INR in people taking warfarin.

When to see a doctor

Talk to a doctor before starting — or instead of supplementing — if you have:

  • chronic fatigue and muscle weakness lasting more than 4 to 6 weeks with no clear cause
  • kidney disease or you're on dialysis (carnitine is sometimes given therapeutically there, under supervision)
  • thyroid disease or you take levothyroxine
  • warfarin or a similar anticoagulant
  • dark urine, severe weakness or an irregular heartbeat after training — that is not a supplement problem

Blood carnitine can be measured. If you suspect a deficiency, a lab test is cheaper and smarter than six months of guessing with capsules.

The short version

  • L-carnitine shuttles fatty acids into mitochondria, but a healthy person already has enough — more doesn't mean faster.
  • Raising muscle carnitine requires 2 g twice daily with 80 g of carbs per dose for 12 to 24 weeks; for most people that isn't worth it.
  • For fat loss: about 1.2 kg on average over several months, mostly in overweight people who were dieting anyway.
  • The most convincing benefit is recovery: 1 to 2 g per day for at least 3 weeks reduces soreness.
  • Don't exceed 3 g per day, and check with a doctor if you have thyroid or kidney issues or take warfarin.
