Turmeric: what it actually does, what dose, and whether it is worth it
What curcumin really does for joint pain and recovery, how many milligrams a day you need, why absorption matters, and when to skip it entirely.

Turmeric is one of the few plant supplements with a decent pile of clinical trials behind it, and also one of the most oversold. The marketing sells it as a fix for everything from joints to immunity; the real picture is narrower and far more specific. Here is what it does, how much you need, and who should actually bother.
Turmeric is not curcumin
Turmeric is a spice — the ground root of the plant. Its active compounds are called curcuminoids, and they make up roughly 2–5% of the dry spice by weight. In practice: one teaspoon of turmeric (about 3 grams) gives you somewhere around 60–150 mg of curcuminoids.
Trials that show an effect use 500–2,000 mg of curcuminoids per day. To hit that from the spice jar you would need to eat 15–40 grams of turmeric daily. Nobody does that. So turmeric in your food stays a spice, not a treatment — if you want the effects described in the research, you are talking about an extract in a capsule.
What it actually does
Joint pain
This is where the evidence is strongest. Meta-analyses of randomised trials in knee osteoarthritis consistently show reduced pain and stiffness at doses of 1,000–1,500 mg of curcuminoids per day for 4–12 weeks. A handful of head-to-head trials against ibuprofen (1,200 mg per day) found a similar effect on pain with fewer stomach complaints.
The caveats matter: those trials are mostly small (60–150 participants), short, and some are industry funded. The effect is real but moderate — on the order of 1–2 points on a 0–10 pain scale. Enough to notice, not enough to replace treatment your doctor prescribed.
Inflammation and training recovery
Some trials show CRP and IL-6 dropping after 8–12 weeks of use. For post-training soreness (DOMS), doses of 150–1,500 mg per day around hard sessions cut subjective soreness by roughly one point on a 0–10 scale over the first 24–72 hours and slightly speed up the return of strength.
Translated to your training: you will notice it after an unusually brutal day — say 5×5 squats after three weeks off — not during a routine week where you never got deeply sore anyway.
What it does not do
It does not build muscle, change body composition, detox your liver, or cure chronic disease. If you sleep 5 hours, eat 0.8 g of protein per kilogram and train without a plan, curcumin is not your bottleneck — and no dose will change that.
Dosing, concretely
- 500–1,000 mg of curcuminoids per day is a sensible range for recovery and general aches; 1,000–1,500 mg per day if you are targeting joint pain.
- Split it into two doses, morning and evening — it clears from the blood quickly.
- Take it with a meal containing at least 10–15 g of fat. Curcumin is barely water soluble; on an empty stomach you lose a good chunk of it.
- Read the label: 1,000 mg of turmeric powder is not 1,000 mg of curcuminoids. Look for a standardisation figure (usually 95% curcuminoids).
- Give it 8–12 weeks, then judge. If after 12 weeks you feel no difference in pain or recovery, stop — there is no point paying for something that is not working for you.
The absorption problem
Plain curcumin is absorbed poorly — only a small fraction of what you swallow reaches the bloodstream. That is why nearly every serious product includes something to fix it: piperine (black pepper extract, typically 5–20 mg) raises blood levels several fold, and phospholipid or fine-particle formulations achieve something similar without the pepper.
There is a flip side: piperine also slows the breakdown of other drugs. If you take any regular medication, that alone is reason to check with your doctor or pharmacist before starting.
Side effects and interactions
- Gut: nausea, loose stools and reflux are the most common — mostly above 1,500 mg and on an empty stomach.
- Blood clotting: curcumin mildly slows platelet aggregation. With warfarin, newer anticoagulants or aspirin, do not add it on your own.
- Gallbladder: it stimulates bile release, which can trigger an attack if you have gallstones.
- Iron: it binds dietary iron. If your ferritin is low, keep curcumin and iron-rich meals at least 2 hours apart.
- Liver: rare but documented — cases of liver injury have been reported, mostly with high-absorption products taken for months.
- Surgery: stop 2 weeks before any planned procedure.
When to see a doctor
- Joint pain lasting more than 6 weeks, or a joint that is swollen, red and warm.
- Morning stiffness longer than 30–45 minutes, or pain in several joints at once — that needs investigating, not a supplement.
- You take anticoagulants, diabetes medication, liver treatment or chemotherapy.
- Yellowing of the eyes or skin, dark urine, persistent fatigue or pain under the right ribs after starting — stop and get checked immediately.
- Pregnancy and breastfeeding — supplement doses are not well studied, though the spice in food is fine.
So is it worth it
Yes, in a narrow set of cases: chronic knee or hip pain where you want something alongside physio and strength work, or heavy training blocks that reliably leave you excessively sore. In that situation it is a cheap, reasonably safe three-month experiment.
It is not worth it if you expect gym progress, weight loss, or protection from disease. For all of that, sleep, 1.6 g of protein per kilogram of bodyweight and consistent progressive overload pay off far more than any capsule.
The short version

- Best evidence is osteoarthritis pain: 1,000–1,500 mg per day for 4–12 weeks, with a moderate effect.
- Take it in two doses, with a meal containing fat, and with piperine or an absorption-enhanced formulation.
- Watch out for anticoagulants, gallstones, low ferritin and planned surgery (stop 2 weeks before).
- Trial it for 8–12 weeks; if nothing changes, stop — it is not a drug and it does not build fitness.
