Spirulina: what it actually does, the dose, and whether it is worth it

What spirulina actually does according to clinical trials, the dose that makes sense, how long a fair test run takes, and when to ask a doctor first.

Spirulina has been sold as a superfood for three decades, and most people taking it could not tell you what they are supposed to feel. The truth sits in the middle: a handful of effects have genuinely been measured in clinical trials, and a lot of claims have nothing behind them. Here is what it does, at what dose, and when it is actually worth your money.

What spirulina actually is

Spirulina is not an algae in the botanical sense but a cyanobacterium (Arthrospira platensis), grown in shallow ponds, filtered and dried into powder. Dry weight is roughly 55–70% protein, plus beta-carotene, iron, some B vitamins, and the blue pigment phycocyanin, which is probably responsible for most of the measured effects.

The thing almost everyone misses: the usual dose is 3 grams per day. Every impressive number quoted per 100 g gets divided by 33. That is why spirulina cannot be a protein source or an iron source, no matter what the label says.

What the trials actually show

Blood lipids

This is the best-documented effect. Meta-analyses of randomised trials, mostly using 1–8 g per day for 4–12 weeks, show LDL cholesterol dropping by roughly 8–12 mg/dL and triglycerides by 15–25 mg/dL. The effect is largest in people who already have elevated values and close to zero in people with a clean panel. For comparison, a standard-dose statin lowers LDL by 30–50%, often 40–70 mg/dL. Spirulina is an add-on at the margin, not a replacement.

Blood pressure

At doses of 2 g per day and up, trials record a fall in systolic pressure of about 4–5 mmHg and diastolic of 2–3 mmHg, again mainly in people who already run high. That is roughly what you get from losing 4–5 kg or halving your salt intake. Useful, but nowhere near enough if you are sitting at 160/100.

Allergic rhinitis

Several smaller trials using 2 g per day for 12–24 weeks recorded less sneezing, runny nose and itching. Sample sizes are small, typically 50–150 people, but the direction is consistent. This is one of the few areas where people subjectively notice a difference.

Training and endurance

Here the evidence is weakest. There are a couple of tiny trials, under 20 subjects, where 6 g per day for 4 weeks extended time to exhaustion in running by a few minutes. There is no demonstrated effect on strength, hypertrophy, or recovery between sets. If your goal is 5 kg more on your squat or two more reps on the bench, spirulina does nothing.

What spirulina does not do

  • It is not a protein source. 3 g of powder gives about 1.8 g of protein. One egg has 6 g, a scoop of whey about 24 g. To get 20 g of protein from spirulina you would need around 33 g of powder per day, which is over 60 tablets of 500 mg.
  • It does not fix B12. Most of the B12 in spirulina is pseudovitamin, an analogue your body cannot use. If you are vegan, you still need a real B12 supplement.
  • It does not detox anything. Your liver and kidneys do that job. The heavy-metal binding studies were done in animals at doses unrelated to what you are swallowing.
  • It is not a weight-loss aid. Trials that record any weight loss see 1–2 kg over 12 weeks, and almost always alongside a calorie-deficit diet.
  • It will not fix anaemia. 3 g provides about 0.9 mg of iron. Daily needs are 8 mg for men and 18 mg for women of reproductive age.

Dose and how to take it

  1. Start at 1 g per day for 3–4 days to see how you tolerate it. Nausea and bloating are most common in the first week.
  2. Move up to 3 g per day, which is 6 tablets of 500 mg or roughly one level teaspoon of powder. There is no clear extra benefit above 5 g for lipids or blood pressure.
  3. Take it with food, not on an empty stomach. Mix the powder into something strongly flavoured; on its own it tastes like a pond.
  4. Give it 12 weeks. Get a lipid panel before and after, or measure blood pressure three mornings a week. If nothing moved, stop.

Practical maths: 3 g per day is about 90 g per month, or a bit over 1 kg per year. Powder is usually 2–3 times cheaper per gram than tablets.

Quality and contamination

This is the one area where spirulina can genuinely be a problem. Poorly controlled cultivation can be contaminated with other cyanobacteria that produce microcystins, which are liver toxins. On top of that, spirulina readily takes up lead, mercury and arsenic from the water and growth medium.

  • Look for a product with independent, batch-level lab testing for microcystins and heavy metals.
  • Avoid products made from wild-harvested blue-green algae from lakes. That is a different organism and the microcystin risk is considerably higher.
  • Powder with no stated origin and no testing is not worth the money you save.

Who should avoid it

  • Phenylketonuria (PKU) — 3 g of spirulina carries roughly 100 mg of phenylalanine. Avoid entirely.
  • Autoimmune conditions — phycocyanin stimulates parts of the immune system, and there is no safety data for conditions like Hashimoto's, lupus or MS.
  • Warfarin therapy — spirulina contains vitamin K and can shift your INR.
  • Gout and haemochromatosis — purine load and extra iron respectively.
  • Pregnancy and breastfeeding — not enough data, skip it.

When to see a doctor

Get medical attention if you develop yellowing of the skin or eyes, dark urine, pain under the right rib cage, or sudden fatigue after starting spirulina. That can indicate liver injury from a contaminated product and needs to be worked up promptly.

Also talk to a doctor before you start if you are on anticoagulants or immunosuppressants, have a diagnosed autoimmune condition, or are thinking of using spirulina instead of a prescribed cholesterol or blood pressure medication. One more thing: if you feel tired and are reaching for spirulina because of the iron, get a full blood count, ferritin and B12 checked first. Fatigue has a dozen causes and none of them are solved by guessing.

The short version

  • Real effects are modest: LDL down about 8–12 mg/dL, triglycerides down 15–25 mg/dL, systolic pressure down 4–5 mmHg, mostly in people whose numbers are already high.
  • The dose that makes sense is 3 g per day with food, tested over 12 weeks with measurements before and after.
  • It is not protein, not iron, not B12, and it does nothing for strength or hypertrophy.
  • Choose it purely on independent testing for microcystins and heavy metals; wild-harvested blue-green algae is the riskier option.
  • PKU, autoimmune disease, warfarin, pregnancy and breastfeeding are reasons to skip it or ask a doctor first.
