The tub tells you to take 20 g a day for the first week, then drop to 5. Do you actually need that loading phase, and what happens if you skip it altogether? Creatine is the most heavily studied supplement in the gym, and it is still the one people get wrong on dosing.

What creatine does inside the muscle

For very short, very hard efforts your muscle burns neither fat nor carbohydrate — it burns ATP, and the ATP sitting in the muscle lasts only a few seconds. Creatine phosphate is the backup system that rebuilds it between reps and between sets.

Supplementing raises muscle creatine phosphate by roughly 20 to 40 per cent, depending on how much was there to begin with. The benefit is narrow but real: efforts lasting 5 to 15 seconds at full effort — a sprint, a jump, a heavy set of squats — get a bit more fuel.

So creatine does not build muscle directly. It gives you an extra rep or two per set, and those reps, repeated over months, add up to more total work done. The growth comes from better training, not from the powder. If you are choosing between this and the other popular powder, the difference is the length of the effort each one supports — creatine covers seconds, beta-alanine covers the minute mark.

The dose: 3 to 5 grams a day

The maintenance dose is 3 to 5 grams of creatine monohydrate daily, every day. A rough rule is about 0.03 g per kilogram of body weight, so bigger and more muscular people sit nearer the top of that range. Going above 5 g brings nothing extra once the muscle is saturated — the surplus leaves in your urine.

Timing barely matters. The muscle fills over days and weeks, not in the hour around your session. There are hints that taking it after training is marginally better, but the difference is far too small to build your day around. Take it whenever you will reliably remember: with breakfast, in a shake, it makes no odds.

Don't skip rest days. Stores fill and empty slowly, and cycling on and off has no basis — a month off simply drops you back to baseline, and you then wait four to six weeks to climb back up.

On form: monohydrate. HCl, buffered versions, ethyl ester and the rest cost more, and none has convincingly beaten it. Micronised monohydrate dissolves more easily in water, which is the only difference you will actually notice.

The loading phase: who it's worth it for

The classic protocol is 20 grams a day split into four 5 g doses for five to seven days. That saturates the muscle in about a week.

Without loading, on 3 to 5 grams a day, you reach exactly the same saturation — it just takes three to four weeks. Same destination, different speed of arrival.

Loading makes sense when something is close: a competition in a fortnight, a testing week, the end of a training block where you want everything you can get. Otherwise skip it. Large single doses are the main cause of bloating and stomach upset, and you get through more powder to end up in the same place.

If you do load, split the dose across four servings and take them with food. Five grams in one go on an empty stomach is the usual reason people say creatine doesn't agree with them.

What you'll notice, and what you won't

Creatine is not a stimulant. No rush, no tingling, no sharpening in your head. If that pre-session buzz is what you're after, that's caffeine's job, not creatine's.

What changes shows up in your training log. After a few weeks the same set runs a rep or two longer, or the same weight moves with less strain. The effect is clear but modest: across months of consistent training it amounts to a few per cent more strength and slightly more lean mass than you'd have managed without it.

The scale goes up by 1 to 2 kg in the first weeks. That is water pulled into the muscle cells, not fat, and it does not create a puffy look under the skin. In weight-class sports it is something to plan around; for everyone else it's just a number not to misread.

Safety, myths and non-responders

Monohydrate has the strongest safety record of any training supplement — years of use in healthy adults with no sign of lasting harm to kidneys or liver.

There is one practical catch. Creatine slightly raises blood creatinine, and creatinine is exactly what a doctor measures to judge kidney function, so a result can look worse than your kidneys actually are. Mention that you take creatine before you have bloods done — it's the same misunderstanding that hangs over protein shakes and kidney health.

Cramping and dehydration get repeated year after year, but the evidence doesn't support them; if anything it points the other way. The hair-loss story traces back to one small study that found a rise in DHT and has never been convincingly repeated — we've gone through that one separately.

Not everyone responds. A meaningful minority already sit close to saturation, usually because they eat plenty of meat and fish, so extra creatine does little for them. Vegetarians and people who rarely eat meat tend to gain the most. It's one piece of the broader question of why two people on the same programme progress differently.

When to see a doctor

Ask your doctor before starting if you have diagnosed kidney or liver disease, if you take medication cleared through the kidneys, or if you're being treated for high blood pressure.

For pregnancy, breastfeeding and younger teenagers there isn't enough data to recommend a dose, so that's a conversation with a clinician rather than a decision made off the label.

Get checked if you develop persistent swelling, back or flank pain, a marked change in urination, or if a kidney function result stays raised several weeks after you've stopped taking creatine.
