How Long Can You Go Without Food?
How long the body actually lasts without food, what happens hour by hour, where the physical limit sits, and the signs that mean call a doctor.

The answer to "how long can a person go without food" varies far more than most people expect — anywhere from a few weeks to over a year. The deciding variable isn't willpower, it's how much fat you're carrying and whether you're drinking water. This isn't a fasting guide, just a walkthrough of what happens inside the body once calories stop coming in.
The first 72 hours
The liver holds roughly 80–120 g of glycogen, about 400–500 kcal. That's the only sugar it can release into the bloodstream. Muscles hold another 300–500 g, but that stock is locked to the muscle and never returns to circulation as glucose.
The sequence usually runs like this:
- 0–16 hours: liver glycogen gets burned. Nothing dramatic happens except that you're hungry.
- 16–36 hours: liver glycogen is effectively empty. Gluconeogenesis takes over — the body builds glucose from amino acids, glycerol and lactate. The brain needs around 120 g of glucose per day at this stage.
- 2–4 days: ketones climb. By day three the brain draws about 30% of its energy from ketones, by day seven over 60%. Glucose demand drops from 120 g to 30–40 g daily.
- After 7 days: muscle protein breakdown falls from an early 60–90 g per day to 15–25 g per day. Resting metabolic rate settles 15–25% lower over a few weeks.
Where the real limit is
The most-cited record belongs to Angus Barbieri of Scotland, who spent 382 days without food under medical supervision in 1965–66. He started at 207 kg and finished at 82 kg, living on water, tea, coffee, vitamins and electrolyte replacement. Without those supplements he would not have survived it.
At the other end are hunger strikes. In people of normal body weight, death typically arrives between day 45 and day 75. During the 1981 Irish hunger strike, ten men died between day 46 and day 73. The limit usually coincides with losing 35–40% of body mass, or a body mass index around 13 in men and 11 in women.
The arithmetic explains the gap. A kilogram of fat tissue carries about 7,700 kcal, so 15 kg of fat looks like 115,000 kcal in the bank. The catch is that the body can only pull roughly 31 kcal per kilogram of fat tissue per day — for 15 kg of fat, about 465 kcal daily. Everything above that gets paid for in muscle, including heart muscle. That's why a lean person deteriorates disproportionately faster than a heavy one.
Water is a completely different clock
Without water you're counting days, not weeks: 3–7 days in normal conditions, and as little as 2–3 days in heat or with physical work. You lose 2–3 litres daily through urine, sweat and breathing, and the kidneys need a minimum of about 500 ml of urine to clear waste. On top of that, 20–30% of your usual water intake normally comes from food, so anyone not eating should drink an extra 500–800 ml.
What a long deficit does to your head
The Minnesota Starvation Experiment of 1944–45 remains the clearest picture. Thirty-six healthy men spent 24 weeks on roughly 1,570 kcal per day while walking 35 km a week. They lost about 25% of their body mass, resting heart rate dropped from 55 to 35 beats per minute, and metabolic rate fell around 40%.
The psychological side was worse than the physical: obsessive thoughts about food, irritability, social withdrawal, apathy. During recovery many overate for months and struggled to rebuild a normal relationship with food. The lesson for anyone training: a long, aggressive deficit doesn't only wreck the body.
Training on empty
A short fast of 16–24 hours costs almost nothing in maximal strength — the drop in a one-rep max is usually under 5%. What suffers is volume and work tolerance: reps in later sets fall by about 10–15%, and endurance beyond 60–90 minutes degrades noticeably.
In practice: 4 sets of 5 reps of squats is fine fasted. Twenty sets of hypertrophy work or 90 minutes of intervals is not. If you're in a deficit and want to keep muscle, hold 1.6–2.2 g of protein per kilogram of body weight, a 300–500 kcal daily deficit, a loss rate of 0.5–1% of body weight per week, and 2–3 strength sessions weekly.
Coming back to food
After 5 or more days without food, the danger isn't hunger — it's the first meal. When insulin spikes, phosphate, potassium and magnesium shift into cells and blood levels crash. That's refeeding syndrome, and it can trigger arrhythmias and heart failure.
Hospital protocol starts at 10 kcal per kilogram of body weight per day (5 kcal/kg in high-risk cases), climbing gradually over 4–7 days, with 200–300 mg of thiamine given before the first feed. If you've skipped one or two days, none of this applies to you — just make the first meal 400–600 kcal instead of 1,500.
When to see a doctor
- Immediately, emergency: fainting, confusion, irregular or racing heartbeat, chest pain, muscle cramps with weakness.
- Before any fast: if you have diabetes treated with insulin or sulfonylureas, kidney or liver disease, or take diuretics, lithium, blood pressure medication or warfarin.
- Don't fast without advice: during pregnancy or breastfeeding, under 18, or with any history of an eating disorder.
- Always get checked: unintentional loss of more than 5% of body weight over 6–12 months.
- Fasts longer than 48–72 hours only make sense with supervision and electrolyte monitoring.
The short version
