Triceps heads: how arm angle shifts the emphasis
The long, lateral and medial heads respond to different arm positions — here is what actually changes, and how to build a training week around it.

Here is the question that comes up most: why does the triceps refuse to change after years of rope pressdowns? Usually the answer is not the number of sets but the angle of the arm. The triceps has three heads, only one of them changes length when you raise your arm overhead, and that one carries most of the mass.
Three heads, one difference that matters
The lateral and medial heads both start on the upper arm bone and cross only the elbow. Whatever the shoulder is doing, their length stays the same.
The long head starts on the shoulder blade, just below the shoulder joint, and crosses two joints. It is the biggest of the three, and it does most of the work in making the back of the arm look thick from the side.
So elbow position relative to the torso changes almost nothing for two heads and almost everything for the third.
Why shoulder angle decides the long head
With the arm by your side, as on a pressdown, the long head sits relatively short at the shoulder end. Pull the elbow behind you, as in a kickback, and it is shorter still, loaded hardest exactly where it has least to give.
Raise the arm overhead and the long head is lengthened across the shoulder, so straightening the elbow trains it from a stretched position. The evidence is strongest here: training muscles at longer lengths produces at least as much growth as training them short, and often more. How big that advantage is, and for whom, is not settled.
Which is why a sensible plan keeps both, rather than picking a side.
Arm overhead: work for the long head
- Rope extension from a low pulley, seated with your back to the machine, so the resistance stays honest at the bottom.
- Single-arm dumbbell extension overhead, which makes a left-right difference easy to feel.
- EZ-bar skull crusher with the arms angled slightly back towards your head rather than straight up.
- Incline bench extension at around 45 degrees, with dumbbells or a bar.
Keep these in the 8 to 15 rep range, take 2 to 3 seconds to lower, and use the full range. If you want to bias the bottom of the movement, a one to two second pause in the stretched position does more for you than adding weight.
The usual error is elbows drifting wide as the load climbs. If you have to change where the elbow sits to finish a set, the weight is too heavy, and with the long head that matters more than usual, because the lift quietly turns into half a press.
Elbow by your side: lateral and medial heads
Pressdowns, close-grip bench, dips and the JM press all keep the elbow near the torso. The lateral head gets the most from these, and it takes heavy work well: sets of 6 to 10 are productive because the position lets you load them.
The medial head works in every variation and contributes most near lockout. It sits underneath the other two, so how much of it you can ever see is limited.
The claim that a reverse grip isolates the medial head is thin. Changing your grip mainly changes how much you can lift and where the elbow tracks; it does not move work from one head to another. Nothing switches a head off. You shift the emphasis.
Building the week
For most people, 10 to 16 direct triceps sets a week across two sessions is enough. If heavy pressing is already in the plan, some of that work is done for you: the standing overhead press and close-grip bench leave the triceps tired before you touch a rope.
The simplest split is one movement with the arm overhead and one with the elbow by your side in each of those two sessions, 3 to 4 sets each.
An example. Day A: overhead cable extension 4x10-12, then pressdowns 3x12-15. Day B: close-grip bench 4x6-8, then single-arm overhead extension 3x12-15. When every set hits the top of the rep range with clean technique, add 1 to 2.5 kg. Order within a session follows the same logic as arranging back work: the weakest thing goes first, while you are fresh.
If the triceps is your lagging part, give the plan 8 to 12 weeks before judging it. Measuring the arm relaxed and flexed once a month tells you more than the mirror does.
Elbows, and what angles cannot do
Overhead work and skull crushers are the usual suspects for elbow pain, either at the point of the elbow or on the inner side. Before dropping the exercise, try two lighter build-up sets, ropes and cables instead of a straight bar, and a temporarily shorter range at the bottom. You do not need to take every set to failure; the last set of an exercise is plenty.
One honest limit: the horseshoe shape depends on how long your muscle bellies are and where the tendon begins. Angles change how much each head grows, not where it attaches. And how much of that separation you can see depends on body fat as much as on training.
When to see a doctor
Get elbow pain checked if it lasts more than two or three weeks despite rest, if there is swelling, if you get tingling or numbness in the fingers, or if a set produced sudden pain along with a loss of strength. Pain that wakes you at night is worth a visit rather than a wait.
The short version
