How to choose running shoes: comfort, fit and injury risk
What actually matters in a running shoe: fit, drop, stack height and weight, how long a pair lasts, and the buying mistakes that cost you the most.

You are standing in front of a wall of thirty models, the assistant asks whether your foot rolls inwards, and you have no idea. Most people stop thinking at that point and buy whatever looks good or happens to be discounted.
That is not a disaster, but you can do better. The three things buyers look at most — appearance, the pronation story and the model name — are among the weakest guides available. The list of things that genuinely change comfort and injury risk is shorter than the shop makes it look.
What a running shoe actually has to do
Feel right immediately. The evidence is strongest for exactly this: a shoe that is comfortable on the first run does better, both for satisfaction and for the number of complaints, than one you plan to break in. Running shoes are not broken in anyway — the midsole does not change, the upper relaxes by a millimetre or two, and that is the whole story.
Leave room at the front. Standing up, you want 5 to 10 mm between your longest toe and the end of the shoe, roughly a thumb's width. The foot lengthens and swells while you run, so a black toenail is almost always a sign of a shoe that is too short rather than of bad running form.
Lock the heel, free the toes. The laces should hold the heel and midfoot so the heel does not lift on push-off. The forefoot is the opposite case: it needs to be wide enough for your toes to spread when you load the foot.
Cushion without wobbling. Stand on one leg in the shoe. If you already feel the platform tipping sideways there, it will be worse at kilometre ten when you are tired.
Everything else — foam type, plates, colour, the energy-return story on the box — only matters after those four.
The numbers on the spec sheet, and what each one means
Size and width
Measure your feet in the afternoon or evening, in the socks you actually run in, and measure both. Half a size difference between left and right is common, and you buy for the larger foot. Most runners end up half a size to a full size above their street shoe.
Width is a separate variable and cannot be fixed with length. If your foot spills over the edge of the midsole when you stand, the shoe is narrow no matter how much space sits in front of your toes.
Drop
Drop is the difference in sole height under the heel and under the forefoot, in millimetres. It ranges from 0 to around 12 mm, with most shoes sitting between 6 and 10 mm.
A lower drop shifts load towards the calf and Achilles tendon; a higher drop shifts more towards the front of the knee. Neither is objectively better, but any change of 4 mm or more deserves a few weeks of adaptation — short runs first, then build.
Stack height
Total material under the heel now runs from about 15 mm in minimal shoes up to 40 mm, which is the upper limit under road racing rules. More foam means a softer landing and usually less muscle soreness after long runs.
The price is a higher platform and less stability. The relationship between cushioning and injury numbers is still unsettled, so do not treat a thick midsole as protective equipment.
Weight
A daily trainer in a UK 8 to 9 typically weighs 230–300 g; a racing shoe is 180–230 g. The rough laboratory rule is that every 100 g per shoe costs about one per cent more energy at the same pace.
That is significant over a race and close to irrelevant for someone running three half-hour sessions a week. Heavier shoes also tend to last longer, so lightness is never free.
The upper
The mesh should breathe and carry no hard seams across bony points. Check the tongue too: a thin, unanchored tongue lets the laces dig into the top of your foot on longer runs.
Outsole and terrain
For tarmac and treadmills, smooth rubber with a shallow pattern is plenty. For mud, forest and rock you want deeper lugs, roughly 3–5 mm, a sturdier upper, and on rough ground a plate that shields the foot from stones.
Trail shoes wear down faster on tarmac and handle worse there. If 80 per cent of your kilometres are on road, buy road shoes and accept being a bit more careful on the odd forest path.
Plated shoes
A carbon or plastic plate combined with a very resilient foam improves running economy by a few per cent, mostly for runners moving at a decent pace. Those shoes cost more, wear out sooner and feel less stable, which makes them a poor first pair.
How long a pair lasts, and what it really costs
Most people get 500–800 km out of a pair. Heavier runners, rough ground and thinner midsoles push you towards the lower end. At 20 km a week, that means replacing shoes every six to nine months.
The signal is the foam, not the tread pattern. Once the cushioning collapses, the shoe feels hard and dead, and your calves and knees absorb the difference. Note the purchase date and keep a count of the kilometres — a training log app or a GPS watch will track mileage per pair automatically.
Prices fall into three tiers: entry level, the middle band where most good daily trainers live, and the top end with advanced foams and plates. The step from entry to middle usually buys the most per pound spent; the step to the top buys less and wears out faster. Rather than the sticker price, think in cost per kilometre: divide by 600.
Above roughly 40 km a week, rotating two pairs makes sense — the foam recovers between runs, both pairs last longer, and limited evidence points towards fewer problems. The same logic applies as with any other kit: it pays to ask what fails first before you buy, not what looks best in the shop.
Common buying mistakes
- Buying by your street shoe size. Sizing varies between makers and between models from the same maker. Measure the foot; do not memorise a number.
- Expecting a break-in period. Pressure on the little toe or rubbing at the heel in the shop stays pressure and rubbing on the road.
- Auto-renewing the same model. The same name often hides new foam, a different width and a changed drop. Try it on again.
- Choosing by pronation. Evidence that prescribing shoes by how the foot rolls reduces injuries is weak. Comfort and habit are the better filter.
- One pair for everything. Running shoes are poor platforms for squats and deadlifts, and they wear out faster if you do everything in them.
- Switching shoe type overnight. Moving to a much lower drop or a minimal shoe without four to six weeks of adaptation is a common route to calf and Achilles trouble.
Who it is worth it for, and who it is not
A solid mid-tier shoe is worth it for anyone running regularly, even 15–20 km a week. That is where the comfort difference is real and lasts for months.
Expensive plated racing shoes are worth it for people with actual races in the calendar and a pace where the plate does something. If you run twice a week with no target, that money buys more as a second pair of ordinary trainers.
No shoe fixes a jump in weekly mileage, a weak calf or a foot that is not used to load. Building volume by no more than about 10 per cent a week and a handful of strength exercises do more; for feet and calves, resistance bands in the right strength are enough equipment.
When to see a doctor
New shoes are not a treatment for pain that will not settle. See a doctor if you have sharp, pinpoint pain in a bone of the foot or shin that worsens under load, if it hurts during normal walking, if there is swelling or numbness in the toes, or if nothing improves after 7 to 10 days of reduced running.
Anyone with diabetes, reduced sensation in the feet or a history of stress fracture should not experiment with footwear and mileage on their own.
