You have trained biceps for two years, the arm has gained three centimetres, and the peak still looks flat. The question that follows is always the same: are you doing something wrong, or is this just how you are built? It is partly both, and the line between the two is clearer than most people assume.

What actually sets the shape of a muscle

Every muscle runs from one bone to another through tendons. Where that tendon attaches, and how long the muscle belly is between the two ends, were settled long before your first session in the gym. Training thickens the muscle fibres. It does not move an attachment point and it does not lengthen a belly.

That is why two people carrying exactly the same amount of muscle on the arm can look nothing alike. On one, the biceps belly runs almost down to the elbow and the arm looks full. On the other the belly is shorter and the tendon longer, leaving a two- or three-finger gap between the elbow crease and the muscle. Same tissue, different picture.

Calves are the clearest case. A high gastrocnemius attachment and a long Achilles tendon give you a short, high calf no matter how many calf raises you do. It will grow. It will grow as the same shape, only bigger.

Fibre composition sits underneath all of this. In most people the split between fast and slow fibres lands somewhere near half and half, with plenty of individual variation, and training shifts it only within narrow limits. It affects how well you tolerate high repetitions far more than how you look.

Peaks, six-packs and other things you did not pick

A biceps peak comes from the length and position of the muscle belly, not from concentration curls or anything sold as a peak exercise. As the muscle grows the peak can look more pronounced, because everything is larger, but the shape itself stays put.

The abdominal wall is even more obvious. How many blocks you have depends on the tendinous bands crossing the rectus abdominis: most people have three pairs, giving six, some have four, and in plenty of people the left and right sides sit slightly offset. Your skeleton sets the frame

Shoulder width comes largely from clavicle length, hip width from the pelvis, and forearm, thigh and torso length are simply what you arrived with. That frame decides how the same muscles read on you compared with someone else, and which lifts sit naturally in your body.

Long thighs and a short torso, for instance, almost always mean more forward lean in the squat and a different balance of strength between squatting and pulling. That is geometry, not a technique fault, and the practical fixes are covered in the article on the squat.

In competitive bodybuilding the frame is part of what gets judged. Proportion and structure carry weight alongside sheer size, which is where the split between classic physique and the open category comes from.

Can an exercise steer where you grow

Partly, and far less dramatically than it is usually sold. The evidence is strongest for the idea that regions of the same muscle are not loaded equally by every exercise: the long head of the triceps works harder with the shoulder overhead, and the pectoralis responds differently to an incline than to a flat press. Over years that can shift proportions within a muscle by a few per cent.

What does not work: moving attachments, adding a peak, or stripping fat off one area with exercises aimed at it. Sit-ups do not burn belly fat.

The useful conclusion is simple. Pick exercises that load the muscle through a full range and in more than one joint position, then let time do the rest. A push pull legs split handles that almost by default.

What is still in your hands

Quite a lot. Muscle size is the most changeable part of the whole story, and it is also the part that alters your appearance most. That takes 10 to 20 sets per muscle group per week, spread across at least two sessions, anywhere from 5 to 30 repetitions, with the last reps close to failure.

Then there is prioritising. If your shoulders are narrow, the side delts and upper back get an extra five to eight sets a week while something else drops to maintenance. That is where the impression of width comes from — not from reshaping anything.

Third is body fat, the fastest lever on how you look and the only one that moves meaningfully in either direction within a few weeks.

Fourth is time. It is worth knowing that the same programme does not produce the same result in everyone: on identical set counts, some people gain twice what others do over twelve weeks. That is genetics too, but the kind that governs speed rather than shape. Asymmetry, and when to see a doctor

A mild difference between left and right is the rule, not the exception. The dominant arm is often half a centimetre to a centimetre bigger, and it does not need correcting. Single-limb work can narrow a strength gap, but it will not erase it.

See a doctor when asymmetry appears suddenly without any change in training, when part of a muscle loses size alongside weakness or pins and needles, when a visible lump or dent shows up after a sharp pain mid-set, or when pain lasts more than a few weeks and wakes you at night. Those are questions for an examination, not for a new programme.
