What Happens to Your Body in Extreme Cold
What actually happens to your circulation, muscles, breathing and kidneys below freezing, how fast frostbite sets in, and when cold becomes a medical emergency.

Cold isn't a character test, it's a physics problem your body solves by moving blood around, shivering, and burning fuel. Past a certain point those mechanisms start losing, and it happens faster than most people expect. Here's what actually goes on, in order, and what it means if you train or work outside.
The first few minutes: blood leaves your skin
The moment cold receptors in your skin fire, the blood vessels there clamp down. Skin blood flow can drop from 200–500 ml per minute to under 20 ml per minute. Your body essentially switches off the outer layer to protect the core: heart, lungs, brain, kidneys. That's why fingers, ears and nose go pale first.
Normal core temperature is about 37 °C. Hypothermia starts below 35 °C: 32–35 °C is mild (shivering, clumsiness, fast breathing), 28–32 °C is moderate (shivering stops, confusion, slow pulse), below 28 °C is severe and life-threatening. The gap between I'm freezing and this is a medical problem is about two degrees of core temperature.
Shivering is an expensive furnace
Shivering kicks in once your core drops roughly 0.5 °C. It raises energy expenditure 2–5 fold: from about 70–80 kcal per hour at rest to 250–400 kcal per hour, and over 500 when you're shaking hard. The fuel is mostly muscle glycogen, so after 1–2 hours of hard shivering the tank runs low and the shivering fades no matter how cold it still is.
That's why a cold person who stops shivering is an alarm, not a sign of recovery.
Muscles work worse than you think
Every degree your muscle temperature drops costs roughly 3–5% of explosive power. Drop muscle temperature 4–5 °C and sprint and jump output fall 15–20%, maximal strength 5–10%. Nerve conduction slows too, so activation lags.
In practice, when you're training outside below freezing:
- warm up for 15–20 minutes instead of the usual 8–10;
- add 2–3 extra ramp-up sets at 40–60% of your working load;
- take 5–10% off the bar on the first day of a sharp temperature drop;
- keep rests under 2–3 minutes or stay covered, because you cool down faster than you recover.
Once hand skin drops below 15 °C, grip and fine motor control noticeably degrade; below 8 °C you won't reliably zip your own jacket.
Breathing and lungs
Cold air is dry air. In a lot of people it triggers bronchoconstriction 5–10 minutes into hard effort: coughing, chest tightness, the feeling that air isn't going in. Among endurance athletes who train in the cold it affects up to half of them.
The thresholds are simple: below -15 °C skip intervals and sprints and keep it easy and continuous; below -20 °C move the session indoors. Nose breathing plus a thin mask or buff over your mouth raises inhaled air temperature by 10–20 °C and keeps some moisture in.
Heart and blood pressure
Cold on its own raises systolic pressure by 10–20 mmHg and narrows coronary arteries. Add a sudden unwarmed effort and you get the classic combination behind snow-shovelling heart attacks. If you have high blood pressure or known heart disease, treat shovelling like a workout: 10 easy minutes to start, a break every 10–15 minutes, a smaller shovel, and push the snow instead of lifting it.
Kidneys: cold diuresis
When blood pulls in from the periphery, central blood volume rises and your body concludes it's carrying extra fluid, so it dumps it. Urine output can rise 2–3 fold in the first two hours in the cold. At the same time thirst drops by about a third, and you're still sweating under three layers. The result is dehydration with none of the usual warning signs. Drink 400–500 ml of warm fluid per hour of activity, in sips.
Water pulls heat 25 times faster
Falling into cold water follows a sequence. The first 1–3 minutes are cold shock: a reflex gasp, hyperventilation, a spike in heart rate and blood pressure. Most drownings happen right there, before hypothermia even starts. Remember the 1-10-1 rule: one minute to get your breathing under control, ten minutes of useful strength in your arms, one hour before you lose consciousness. Wet clothing in wind on dry land works the same way, just slower.
Frostbite
Tissue freezes at about -0.5 °C. At a wind chill of -28 °C, exposed skin is damaged in roughly 30 minutes; at -40 °C it takes 10 minutes or less. First degree is white, numb skin with no blisters and heals clean. Second degree produces clear blisters within 24 hours. Deeper injury shows dark, blood-filled blisters and needs a hospital.
How to rewarm, and how not to
- Get wet clothing off before anything else.
- Rewarm in water at 37–39 °C for 15–30 minutes, until the skin flushes.
- Don't rub the frozen area with your hands or with snow, and don't hold it over a fire or radiator, because numb skin burns easily.
- Don't thaw a body part if there's any chance it refreezes before you reach shelter.
- Alcohol dilates skin vessels and speeds up heat loss, however warm it feels at first.
- With hypothermia expect afterdrop: the core can fall another 1–2 °C after you get someone inside, so rewarm gradually and don't move them roughly.
When to see a doctor
- Skin stays white, waxy or hard, or is still numb after 30 minutes of rewarming.
- Blisters appear, especially dark or blood-filled ones.
- Confusion, slurred speech, stumbling, or shivering that stops while it's still cold; that's emergency services, not wait-and-see.
- Chest pain, pressure or shortness of breath during effort in the cold.
- Cough and chest tightness lasting more than an hour after you stop, or recurring every single time.
- Fingers that regularly turn white then blue in the cold and hurt on rewarming; get that checked.
The short version
- Your body sacrifices skin and limbs to hold the core at 37 °C; hypothermia starts below 35 °C, and shivering stops below 32 °C.
- Shivering burns 250–500 kcal per hour and empties glycogen in 1–2 hours, so cold fatigues you like a training session.
- A cold muscle loses 3–5% of explosive power per degree; warm up 15–20 minutes and add 2–3 ramp-up sets.
- Drink 400–500 ml of warm fluid per hour, because cold diuresis and blunted thirst work against you together.
- Below -15 °C skip intervals, below -20 °C go indoors; and shivering that stops plus confusion means call for help.
