What happens to your heart during and after training, how many minutes a week actually change it, and which symptoms mean you should stop and see a doctor.

How Training Affects Your Heart

Your heart is a muscle, and like any muscle it responds to how much you load it. Training does not wear it out — it changes its size, elasticity and efficiency so it needs fewer beats to do the same job. Here is what actually happens, how much work it takes, and when a symptom is more than fatigue.

What happens while you train

The moment effort starts, your heart raises two things: how often it beats and how much blood it pushes out per beat. At rest it moves about 5 litres of blood per minute. At maximum effort an untrained person reaches 20-22 litres; a well-trained endurance athlete goes past 35.

Blood pressure behaves predictably during running or cycling: the top number (systolic) climbs from around 120 to 160-200 mmHg while the bottom number stays flat or dips slightly. That is a normal response. Lifting is different — during the last two reps of a heavy set, pressure can briefly spike above 250/150 mmHg, especially if you hold your breath. A healthy heart handles this and values return within seconds.

What changes after 8-12 weeks

  • Resting heart rate drops 5-15 beats per minute. An untrained person usually sits at 70-80; after three months of consistent cardio, often 58-65.
  • The left ventricle widens slightly and its wall thickens — more blood per beat, fewer beats for the same work.
  • Blood pressure falls by an average of 5-8 mmHg systolic in people who start out elevated. That is roughly the effect of a mild medication, without the prescription.
  • VO2max — how much oxygen your body can actually use — rises 10-20% in beginners over three months.
  • Heart rate recovery: one minute after you stop, your pulse should fall at least 12 beats. In trained people it drops 25-40. It is the best free progress marker you have.

How much is enough

The number that shows up in every guideline: 150 minutes of moderate cardio per week, or 75 minutes of hard cardio, plus two strength sessions. It does not have to be in one block — 22 minutes a day adds up to the same 150, and so do three 50-minute sessions.

If you currently do nothing, the biggest return is at the start. Going from zero to 60 minutes a week cuts risk more than going from 150 to 300. The first hour is the most valuable hour you will ever add.

Getting the intensity right

The 220-minus-age formula is crude; it misses by roughly 10-12 beats in either direction. A slightly better one is 208 minus (0.7 × age): at 50 that gives 173 instead of 170, at 30 it gives 187 instead of 190.

If you have no monitor, use the talk test:

  • Easy: full sentences without pausing, but you would not sing. Around 60-70% of max heart rate. Most of your weekly volume lives here.
  • Moderate: sentences of 4-5 words. Around 70-80%.
  • Hard: two or three words, and you cannot hold it past a few minutes. 85% and up.

The most common mistake is that everything lands in the middle — too hard to build a base, too easy to be a real stimulus. Aim for 70-80% of your minutes genuinely easy and the rest genuinely hard.

A concrete week

  1. Monday — 40 minutes of easy running, brisk uphill walking or cycling at 60-70% of max heart rate.
  2. Wednesday — full-body strength: 3 lower-body exercises, 3 upper-body, 3 sets × 8-12 reps each, 90 seconds rest.
  3. Friday — intervals: 10 minutes warm-up, then 4 × 4 minutes hard (85-90%) with 3 minutes of easy movement between, then 5 minutes cool-down. About 45 minutes total.
  4. Saturday — 50-60 minutes at an easy, conversational pace.

That is around 160 minutes and two quality stimuli. Do intervals once, at most twice, a week — your heart adapts faster than your tendons and joints, which is exactly how people overdo it.

Lifting, breathing and pressure

Strength training is good for your heart, but breathing matters. Holding your breath through an entire set drives the pressure spike highest. On normal working sets of 8-12 reps, exhale through the hard part of the movement. If your blood pressure is elevated, stay in the 10-15 rep range with weights you fully control, skip true one-rep maxes, and avoid positions where your head sits below your heart.

If your ears ring, your vision spots, or the room tilts after a set, the weight is not the problem — your breathing is.

What is normal and what is not

Normal: a pounding chest after intervals, mild breathlessness for 5-10 minutes afterwards, and a resting pulse 5-8 beats higher the morning after a hard session. It is also normal to occasionally feel a skipped beat, especially after poor sleep or three coffees.

Not normal: chest tightness or pressure during effort that felt easy last month, blacking out while exercising, a pulse still above 120 after ten minutes of rest, or a sudden unexplained drop in how much effort you can tolerate.

When to see a doctor

  • Emergency, now: chest pain, tightness or pressure lasting more than a couple of minutes, spreading to the jaw, neck or left arm, with cold sweat or nausea.
  • Same or next day: fainting or near-fainting during effort, or palpitations lasting several minutes with dizziness.
  • Before starting a program: if you are over 40 and have not trained in years, if your blood pressure regularly reads above 140/90, if a close relative died suddenly of heart problems before 50, or if you take heart, blood pressure or thyroid medication.
  • Book a check-up: resting pulse consistently above 100 or below 45 without an explanation, swollen ankles, or breathlessness walking up a single flight of stairs.

The short version

  • 150 minutes of moderate or 75 minutes of hard cardio per week, plus two strength sessions — that is the whole requirement.
  • In 8-12 weeks expect a resting pulse 5-15 beats lower and systolic pressure 5-8 mmHg lower if it started high.
  • Keep 70-80% of your minutes genuinely easy (full sentences) and the rest genuinely hard; the middle gives the least back.
  • Track how far your pulse falls one minute after stopping: under 12 beats is a weak sign, 25 or more is good.
  • Chest pain on exertion and fainting during exercise are never rest-day problems — they are appointment problems.
